Y Whittlesea is a not-for-profit that happens to be part of the oldest youth organisation in the world and oldest social enterprise where all profits go back into our communities. Our Inclusion Services provide a wide range of programs and supports for children and adults with a disability, families and the local community. We offer support coordination services in Melbourne, Horsham and surrounding areas. What’s the opportunity?
As a Support Coordinator, you’ll play a vital role in helping individuals with disabilities navigate and access the services and support they need to live independently and achieve their goals. You’ll focus on addressing barriers and reducing complexities for our participants in a supportive environment, using a strengths-based approach. Some of the key duties include:

Hours: This is a part-time opportunity, ideally 0.6 FTE equivalent over 3 days. As a flexible workplace, we’re open to negotiating hours for the right person, such as working 5 shorter days across the week – with the potential to increase hours and days as your case-load increases.
Location: Our Horsham office is located in the Centre for Participation. This role is open to Horsham and surround (Ararat, Stawell, etc.), with office attendance negotiable depending on location.
